The Lexicon doesn't list one. (I wanted to use one too.) He just has wizard listed as a first name. (I wanted to guess that Theodore was named for him, but decided it was too much of a risk.) So I used Nott the elder. (He's the opposite of Alice whose maiden name is unknown.)
